CANCELLATION POLICY:
Effective January 20th 2023
Due to a high no call/no show rate, a $35 reservation fee is required to reserve an appointment for those that have missed appointments with us in the past and failed to notify us at least 24 hours in advance. This reservation fee will be applied to the invoice for the next booked appointment. The credit will remain on your account if you cancel or reschedule your appointment 24 hours in advance. This $35 fee is forfeited if you are a no call no show for the appointment, or reschedule or cancel the appointment less than 24 hours in advance. Please call, text, or email us if you need additional clarification.
​APPOINTMENTS:
We encourage our clients to book wellness and sick appointments online. After requesting an appointment our staff will review your request and will call, text or email you for additional info. After we've spoken with you, you will receive a confirmation email with additional forms to complete prior to your appointment. Those forms are also linked below.
Please make sure to leave a telephone number when booking an appointment. If we are unable to reach you to confirm the visit, your appointment request will be cancelled 48 hours before the date, so we can offer it to another family in need of care.

Please note that we are an affordable care clinic, not a free clinic. a $38 office visit fee is due at the time of services. If you are in need of a financial assitance for your visit, please see our community resource page.

Please note: When requesting an appointment, if the date you are requesting has a dash listed on that date, there are no available appointments for that day.

SURGICAL APPOINTMENTS:
We require a surgical consultation prior to the day of surgery to establish a relationship with you and your pet, and to ensure your pet is healthy for the procedure. The cost of this visit is $38+tax.

After this visit, the Doctor will provide an estimate of services for the procedure. Your pet's surgical appointment will be booked the day of the consultation, or one of our team members will call you to book.

We perform surgeries on Wednesday and Thursday morning's. Please note, we are unable to do emergency surgeries due to our small staff and solo doctor.

PAYMENT:
Payment for the entire medical plan is required when services are rendered.
Our preferred methods of payment:
Cash or Debit and Credit cards (Visa, Mastercard, Amex, and Discover)
We also accept Care Credit (please note: a small fee is charged for using this service).
We also accept Kentucky Humane Society Vouchers (please visit their website to apply), Shamrock Vouchers, and Louisville Metro Animal Services Surgery Vouchers. These vouchers MUST be with you at the time of your appointment.
